Graduate Program Forms
CVM M.S. and Ph.D. Students will use the following forms during their academic programs:
Committee Request Form (completed during M.S. student’s first semester, Ph.D. student’s second semester)
Committee Change Form (completed as changes are made)
Graduate Program of Study (completed during M.S. student’s first semester, Ph.D. student’s second semester)
Graduate Program of Study – Continuation
Graduate Program of Study – Attachment Sheet (completed if class are to be taken but not on Program of Study)
Graduate Program of Study Change Request (completed as changes are made)
Ph.D. Admission to Candidacy Form (Form should be completed and signed the day Ph.D. Oral Examination is passed)
Research Proposal Template - Also available in Word format (submitted by M.S. and Ph.D. students before 2nd half of program is completed)
Report of Examination Results (request form before M.S. defense and Ph.D. preliminary exams and final defense)
Transfer Approval Form (used to transfer previous graduate work to MSU)
*All forms should be completed or reviewed for accuracy by the Coordinator of Research and Graduate Studies before required signatures are obtained. Other MSU Graduate School Forms can be found at the MSU Office of Graduate School Forms site as needed.
